The vast majority of nursing homes are institution centered and are based on a medical model. What does that mean? Residents Follow the schedule of the nursing home. You eat when they want you to eat. You bathe when they want you to bathe. You may have to get ready for bed when they want you to go to bed. The resident's life revolves around the schedule of an institution. In many ways, that is no different than when we send someone to prison. Another example is being in the hospital. Would you like to spend the rest of your life in a hospital?
The Green House movement was started by Dr. William Thomas. The Green house movement is resident centered. In theory and general practice, the Green House is a small home that can house 6-12 residents. Residents live in individual rooms. They have individual showers. Some homes are equipped with individual lifts in each room to take the resident from the bed to the shower. Many have large great rooms with a fireplace. Each state has different requirements for staff and sometimes this means a state has to e0 and their definitions for staff. Larger institutions have made Green Houses within an existing nursing home. Each wing, spoke or floor of an existing is turned into individual Green Houses.
The Green House movement may not be the definitive future of nursing homes but it is moving us in the right direction. The emphasis is on the word home instead of nursing.
